St. Helens Council is developing a new service, which invites individuals to complete a Self Assessment Application.

Giving you the choice

Self Assessment gives people the choice to assess their own care needs or carry out an assessment on behalf of someone else.

This service is aimed at adults who are finding looking after themselves increasingly difficult. For example you or someone you know may be having difficulties maintaining their property, accessing benefits or keeping warm in winter.

Putting you in touch with the right help

There are services available in your local community that may be able to assist you with your identified difficulties. After you complete a self assessment you will be contacted by one of our team to discuss the next steps which could be providing information or arranging a visit to assist you in person.

If it seems your needs are more complex and we think you could benefit from a full assessment we will tell you this.

Who can use this service?

Self assessment is available to people who live within St. Helens Council's boundary, and are 18 yrs and over.
