We are currently looking to recruit two temporary full time Assessment & Review Worker into the Later Life, Disability, Mental Health, DOLS and All Age Review Team. The team is based at Forster House and consists of Social Workers and Assessment and Review Workers. The team has a number of functions including managing the reviews across all adult services.
The role of the Assessment and Review Worker is to arrange and manage reviews for individuals who receive support provided by the Local Authority. This will include individuals supported within a variety of settings including in their own home, in residential or nursing care, within sheltered accommodation and supported living settings. The reviews will ensure the support provided continue to meet the individuals care needs as defined under the Care Act 2014.
This is an excellent opportunity to gain and develop experience within a Social Work setting. The team is a supportive team and the Assessment and Review Workers will work alongside Social Workers and Managers. A good induction and monthly supervision will be provided as well as ongoing peer support.
