An exciting opportunity has arisen for a qualified care manager/senior care manger within the Later Life & Memory Service working within the care management division of Adult Social Care & Health.
The team comprises of both social care and health staff which facilitates multi-disciplinary and joint working.
You must have substantial experience of working with older people experiencing functional ill health and dementia. You will have a professional qualification in social work. An Approved Mental Health Practitioner qualification is desirable or a willingness to train. You will need to be able to work autonomously, as well as part of a team. You will be able to demonstrate skills in assessment, risk management, planning, commissioning, implementation of support packages and the reviewing process. You will need to demonstrate an understanding of mental capacity assessments, deprivation of liberty provisions and the safeguarding of vulnerable adults. You will have experience of self-directed support within the context of the Health and Social Care Bill 2014. You will demonstrate working in partnership with other statutory and voluntary agencies.
